As part of your MVP Medicare Advantage plan, you can receive delicious, high-quality meals to help you relax and heal after an inpatient hospital stay. Crafted by chefs and registered dietitians, meals are medically tailored to support overall wellness and health concerns that come with a variety of major chronic conditions, including:

- Kidney conditions
In addition, your Mom’s Meals menus include options to fit your lifestyle and health needs, such as:

- Low-sodium foods

How Mom’s Meals Works
An MVP Case Manager will call you once you’re discharged from the hospital. We’ll check on how you’re doing and arrange for you to receive an order of Mom’s Meals. Next, Mom’s Meals will call you to make your meal selections and ensure they get delivered to your home.
You’ll get a delivery of 14 refrigerator-safe meals to your home. Meals are packaged for storage in the fridge for up to 14 days. Heat, eat, and enjoy in minutes!
For UVM Health Advantage Plan Members
If you’re a UVM Health Advantage plan member, and you have an inpatient or observation stay for a congestive heart failure, diabetes, or a behavioral health diagnosis, you can receive 14 meals every seven days for 12 weeks.
